Before you sign up, please read these terms and conditions.
Fees:
1. The fee is $225 US for the Intermediate Metabolic Typing Questionnaire, a one-hour results session, a complete results packet, and a half-hour follow-up session, within 1-2 weeks of the results session. 2. The fees for follow-up sessions are $80 US per one-hour session. Additional follow-up sessions and e-mails numbering more than 5 between sessions are billed at $25 per 1/4 hour. 3. Please budget $100-$200 for Ultralife Supplements based on your Metabolic Type. The initial supply can last from 1-3 months, depending on the individual and the quantities ordered. 4. Payment is made in advance by check or by Pay Pal.
Instructions
1. After your sign-up and payment have been processed, you will be sent an e-mail with instructions on how to take the test, where to find the test, and my Advisor ID number, a link to the patient information form and the client procedure list. When you submit the Patient Information Form, you will then go to the Healthexcel website to complete the Intermediate Questionnaire. Please set aside one hour to complete the patient information form and to complete the questionnaire. Please make a copy of the questionnaire and save it on your computer. 2. You will be asked before submitting the Patient Information Form to read this disclaimer. (link to disclaimer) 3. Once your test results have been submitted, they will be sent to Amy Andrews within 24-48 hours. Amy will contact you to schedule a results session (one hour) once you have received your results materials via overnight courier. 4. At the end of the results session, we sill schedule a half-hour follow-up. 5. Five e-mail inquiries are included between consultations. The sessions may be held in person at Andrews Integrated Health in Northbrook, Illinois, or on the phone.
Terms and Conditions
1. There are no refunds.
2. Cancellations are not advised. The program is most effective executed as prescribed. 3. Because your time is scheduled specifically for you, please allow 48 hours to reschedule an appointment. 24-Hour notice is required to reschedule, and less than 24 hours may require a cancellation fee.
Paying by Check
Please call or contact Amy Andrews at 847-412-0744, ext. 3, and she will give you instructions to mail a check. You will then be given the link for the Patient Information Form, be asked to read the disclaimer, and be given the Healthexcel Advisor Certification # in order to make your test valid.
